Maryland Living Well Center of Excellence - Division of MAC Inc is offering two evidence-based programs for people looking for help managing your asthma, arthritis, diabetes, hypertension, congestive heart failure, COPD, kidney disease, or other ongoing health problems.

ABOUT THE CLASSES:


DIABETES SELF-MANAGEMENT

What is the Class?

Diabetes Self-Management Program (DSMP) is a workshop to help people manage their diabetes. This workshop is recommended for adults with Type 2 Diabetes. Adult-age caregivers, family members, and others members of a support team are highly encouraged to attend. This interactive workshop is designed to improve an individual’s A1C’s and reduce the complications associated with Diabetes.



The content of this program is based on focus groups with diabetes educators and people with diabetes, and meets the content standards of both the American Diabetes Association and the American Association of Diabetes Educators.


What Can I Expect During Class?

DSMP, a community-based class, is an interactive and participatory program for adults who are living with Type 2 Diabetes. Every participant is provided a FREE book mailed to them once they attend the first session. To learn more, visit the Maryland Living Well Center of Excellence website.

CHRONIC DISEASE SELF-MANAGEMENT

What is the Class?

Chronic Disease Self-Management Program (CDSMP) is for adults with a variety of chronic health conditions. The program may be particularly beneficial for people who have more than one health condition, whose health problems have begun to interfere with their valued life activities, or who have had difficulty following their health recommendations. Caregivers are welcome at the workshops and are encouraged to participate as well.


What Can I Expect During Class?
