What is an Advance Directive?
A document expressing your wishes about critical care in the event you are unable to speak for yourself. It gives you the power to make future decisions about your critical care, including who to designate as a Health Care Agent. Your family and friends are relieved of the burden of having to make decisions without knowing your choices in a highly stressful situation.
What is a Health Care Agent?
Someone you trust.
Chosen in advance to make decisions on your behalf.
Someone willing to ask questions of the health care team and advocate for your wishes.
