Up to 150K in mini-grants available

The Mid-Shore Health Improvement Coalition is making mini-grants available for the purpose of improving the access and delivery of health services, reaching underserved and vulnerable populations, and integrating clinical and social support services within the five-county mid-shore region. Applicants can request between $5,000-$15,000 in funding. Approximately $150,000 in total funding is available. Projects should focus on one or more Coalition priority areas which include:

1. Diabetes/Nutrition/Physical Activity

2. Tobacco/Cancer

3. Social Determinants of Health

4. Health Literacy

5. Provider Recruitment/Retention

6. Telehealth
